DentalForms provides downloadable dental form templates for dental practices. By using our service, you agree to these terms.
When you purchase from DentalForms, you receive downloadable PDF form templates. These are blank templates — you are responsible for filling them out and using them in compliance with applicable laws and regulations, including HIPAA.
Upon purchase, you receive a non-exclusive, non-transferable license to use the form templates within your dental practice. You may not resell, redistribute, or sublicense the templates.
All payments are processed securely via Stripe. Due to the digital nature of our products, all sales are final. If you experience a technical issue preventing you from accessing your purchase, contact us for assistance.
DentalForms provides templates as a convenience tool. We are not a law firm or compliance consultancy. You are responsible for ensuring that the forms you use meet your legal and regulatory requirements. Templates are provided "as is" without warranty of any kind.
DentalForms shall not be liable for any indirect, incidental, or consequential damages arising from the use of our templates. Our total liability is limited to the amount you paid for the specific template(s) in question.
We may update these terms from time to time. Continued use of DentalForms after changes constitutes acceptance of the updated terms.
